Lab Member Annual Lab Safety Requirements
These items need to be accomplished annually by all lab members before the lab safety review.
Annual requirements in the Risk and Safety Solutions Online Tools

- Review your lab’s Hazard Assessment
- Ensure your UC Laboratory Safety Refresher is up to date, check in the Learning Center
Annual Refresher Training
The Annual Refresher Training needs to be documented on a form equivalent to this Annual Refresher Training Form.

- Illness and Injury Prevention Program
- Emergency Action Plan
- Chemical Hygiene Plan
- Safety Net 13 Guidelines for Chemical Spill Control
- Safety Net 8 Chemical Waste Disposal Guidelines
- Or take the Hazardous Waste Management and Minimization class
- Ensure you have been trained on appropriate SOPs when updated and at least every three years
Lab Manager Annual Lab Safety Requirements
The following steps will help you prepare for your annual lab safety review:
Annual Requirements in the Risk and Safety Solutions Online Tools

- Review your Hazard Assessment
- Ensure the lab members have reviewed their Hazard Assessment.
- Ensure the roster is up to date: add or remove lab members in Profile as necessary.
- Review and certify your chemical inventory in Chemicals
- Directions to reconcile a chemical inventory
- Ensure everyone has up to date UC Fundamentals of Laboratory Safety, check in Profile.
- Directions to verify training in Profile
- Annual lab self-inspection in Inspect or paper checklist
- Directions to perform a self inspection
Annual Refresher Training
The Annual Refresher Training needs to be documented on a form equivalent to this Annual Refresher Training Form.

Every three year requirements

- Review Standard Operating Procedures (SOPs)
- Ensure lab members are trained on appropriate SOPs when updated and at least every three years
- UC Laboratory Safety Refresher
DSC Annual Lab Safety Requirements
Annual requirements in the Pre-Inspection Report:

- The roster is up to date and add lab members in Profile as necessary
- The roster in Profile and Hazard Assessment match
- Completion of the UC Fundamentals of Laboratory Safety
- Completion of the PPE Safety Training
- Annual Review of the hazard assessment
- Lab members have reviewed the hazard assessment
- Annual lab self-inspection-Not the Biosafety self-inspection (could have been completed on paper)
- Review and certify the chemical inventory
- The hazard assessment is accurate
- Nobody has expired UC Fundamentals of Laboratory Safety Refresher training
- Nobody has expired other training (biosafety, controlled substances etc.)
Verify Pre-Inspection Findings
Since the Pre-Inspection Report can be several days out of date you can verify in each potential issue in the appropriate Risk and Safety Solutions Online Tool.

- The lab Hazard Assessment has been reviewed by the PI in the last year
- All lab members have reviewed the Hazard Assessment
- The roster is up to date: add or remove lab members in Profile as necessary
- The chemical inventory in Chemicals has been certified
- Directions to reconcile a chemical inventory
- Everyone has up to date UC Fundamentals of Laboratory Safety, check in Profile
- Directions to verify training in Profile
- Annual lab self-inspection is available in Inspect or paper checklist
- Directions to perform a self inspection
